Private Vocational School Annual Report

All private vocational schools are required to submit a student data report, annually, for continued licensure by the Workforce Board. Private vocational schools must submit required data on students trained from July 1, 2008 through June 30, 2009 by 5 p.m. on October 30, 2009.

Each page can be printed out, if needed. Please read all instructions carefully and follow data reporting guidelines.

Data Reporting Form:
The data reporting form is available as an MS Excel file that can be saved to your computer, filled in and uploaded to a secure server (see Data Reporting Instructions). The form is also available as a PDF document that can be downloaded and printed, filled out by hand and mailed. To open PDF files, you will need Adobe Acrobat Reader.

The Annual Report is due October 30, 2009 at 5 p.m.
